Staff Carers has the ability to delete an eForm only if there are no profiles assigned yet and if the eForm is still disabled. 

If there are profiles assigned to the eForm, kindly remove it first.  Please follow the steps in the "Removing Groups or Profiles from an eForm" section of this article first: How does a Staff Carer assign and remove groups/profiles on an eForm?.

If the eForm doesn't have any profiles assigned to it, please follow the steps below on how to delete the eForm.  

1. Log in to CareMonkey.

2. Click My eForms on the left panel of Care Central.

3. Locate the eForm you created and click Edit.

4. The eForm Menu will appear. Click the Delete this eForm.
