Administrators can modify the questions or change the settings of an existing eForm. When any of the following details below is changed, the Administrators and Care Profile Owners (i.e., parents, adult club members, adult participants, employees, etc.) will receive an email notification

Administrators will receive an email notification if changes were made to any of the following:

  • Name of the eForm
  • Description
  • Start time
  • End time
  • Response by date
  • eForm was enabled

Profile Owners will receive an email notification if changes were made to any of the following:

  • Name of the eForm
  • Description
  • Start time
  • End time
