If the organisation requires a Care Profile Owner (i.e., parent, adult club member, adult participant, employee, etc.) to respond to questions in addition to the normal CareMonkey profile questions, this can be done by following the steps below. Note that the information in the Additional Questions section is only being shared with the organisation itself. These cannot be shared with other organisations.
To respond to the Additional Group Questions, please follow these steps.
1. Click the name of the profile.
2. Select ADDITIONAL QUESTIONS from the FULL VIEW drop-down list, or scroll down to the Additional Questions section.
3. Fill out all the form completely and click on Save Response. Required items are presented by asterisks.
4. As a change has been made to the Care Profile, you will need to re-consent to confirm that the profile has been changed and that the medical information contained within is up-to-date.
