This article will show you the step-by-step procedures on how to attach a PDF file to an existing eForm. The attachment will be accessible to Care Profile owners (typically, parents) who'll receive the email notification.

1. Click the three horizontal lines icon of an eForm.

2. Scroll down to the Additional Tools section and click Attachment for User. You can click on this button as many as you want if there's a need to attach more than one file.

3. Click Browse or Choose File to locate and select the file from your computer, and hit Open. Please note that only PDF files can be attached to an eForm. 

You may add instructions for the Profile Owners. 

4. Click Save eForm.

5. If an eForm is enabled, an email notification will be sent to the Profile Owners regarding the changes that transpired.
